We offer Hormone Replacement Therapy and weight loss services to adult clients. Services are available in-person and via telehealth.

Certain services include prescription medications issued by licensed healthcare professionals. Laboratory testing is conducted through third-party lab partners; Vitality Centers NW does not operate its own laboratory.

3. Payments and Subscriptions

All payments are processed in-office or online via Stripe.

Initial consultations require advance payment before the appointment is confirmed.

Ongoing treatment plans may require an active monthly subscription for continued care and access to prescribed medications.

Refunds are not guaranteed and are handled on a case-by-case basis. To request a refund, contact [email protected].

By enrolling in a subscription, you authorize recurring billing according to your selected plan until canceled in accordance with these Terms.

4. Medical Services and Disclaimer

All medical services are provided by licensed healthcare professionals.

We prescribe medications only when medically appropriate and after evaluation.

Vitality Centers NW does not operate its own pharmacy or laboratory but may refer patients to third-party providers for related services.

Our Services are intended for clients who are seeking care within the scope of hormone and weight-management therapies.

5. Privacy and Data Handling

We value your privacy.

We use secure, HIPAA-compliant systems, including DrChrono, for the collection and storage of personal health information (PHI).

Other platforms such as HighLevel, Google Workspace, and Stripe may be used for administrative or payment purposes, but no PHI is stored in those systems.

6. Communication and SMS Messaging

By providing your phone number and opting in to our messaging program, you consent to receive SMS messages from Vitality Centers NW.
