Vitality Centers Northwest ("we," "us") provides this Consumer Health Data Privacy Policy as required by the Washington My Health My Data Act and applicable law. It explains how we collect, use, and share consumer health data, and the rights you have over that data.

What consumer health data we collect

Depending on how you interact with us, we may collect: health assessment and intake responses you choose to submit, including symptoms, health goals, and health history; appointment and consultation requests; communications you send us about your health; and information you provide during care, such as lab results ordered through our providers.

Website browsing data, such as the pages you visit, may indicate an interest in health services. We treat this with the protections described in our Cookies and Tracking Technologies section, and no assessment answers or health details are ever shared with advertising or analytics tools.

Why we collect it

We collect consumer health data to provide the services you request: reviewing your symptoms and goals, scheduling consultations, providing hormone optimization and related care, ordering labs, managing prescriptions, billing, and communicating with you about your care. We do not sell consumer health data, and we do not use it for advertising.

Sources of consumer health data

We collect this data directly from you, through forms and assessments you complete, communications you send, and consultations you attend, and from laboratories and pharmacies involved in your care with your knowledge.

Who we share it with

We share consumer health data only as needed to provide your care and run our practice: with our providers and clinical staff; with laboratories and pharmacies involved in your treatment; with our practice management and communications platform, which processes data on our behalf under contractual restrictions; and with payment processors for billing. We do not share consumer health data with advertising platforms. We may disclose data if required by law.

Your rights

You have the right to: confirm whether we collect, share, or sell your consumer health data; access that data, including a list of third parties with whom it has been shared; withdraw consent you have previously given; and request deletion of your consumer health data.
